Job Description:
Job Description CSO is looking for a Nurse Care Coordinator to
join our Community Partner Program in Pittsfield. The Community
Partner Program is an innovative service provider with a
multidisciplinary team consisting of nurses, Licensed Clinical
Behavioral Health Specialists, Bachelor's and Master's level Care
Coordinators, and Peer Partners. Together we provide individualized
support to adult MassHealth members who have complex medical and
behavioral health needs. We coordinate communication between
behavioral health, substance use, and medical care providers, and
we connect people to services in the community to help them meet
their self-identified health and wellness goals. Responsibilities
Collaborate with CP team members in the development
oftransition-of-care plans for Members who are discharged from
inpatient care andother patients in need of in-home CP nurse
visits. Conduct nursing visits to assigned Members in theirhomes or
other residential settings. Visit shall include brief assessment
andevaluation, provision of planned care, medication
reconciliation, and patienteducation and support to assist them
with self-management of their care. During outreach visit,
communicate with family membersor other caregivers regarding the
patient's care as appropriate and permittedby the patient.
Following (or during) outreach visit, coordinate withCCC and/or CC
as appropriate regarding further care and support for thepatient.
Participate in Care Teams as appropriate for individualMembers
needing ongoing outreach. Provide in-service training for CP staff
on commonmedical issues and appropriate treatment, intervention and
support strategies. Provide case consultation pertaining to medical
issuesfor CCCs and other CP staff as requested. Communicate with
non-CP members of Care Teams (e.g.,hospital staff, PCPs, nursing
home staff) as needed to support and advanceappropriate care for
Members. Conduct medication reviews and monitoring as needed.
Qualifications Must be licensed in Massachusetts as aRegistered
Nurse or Licensed Practical Nurse, and have a minimum of threeyears
of experience treating adults. Related degree preferred. Experience
treatingindividuals with serious mental illness and substance use
disorders preferred.Primary care experience strongly preferred.Must
be familiar with community resources and possess good
communicationand interpersonal skills. Ability to adapt to a
variety of environments andmake decisions when faced with unplanned
circumstances. Ability to establish trusting relationshipsthrough
compassion, empathy and insight.Capacity to be an effective member
of a Team. Demonstrated sensitivity to the needs and perspectivesof
individuals from diverse cultural or linguistic backgrounds.
Computer proficiency required including abilityto work with email,
electronic health record and word processing software. Driver's
license and reliable transportation required. Bilingual candidates
