Healthcare for the Homeless is looking for a compassionate and
self-motivated Advanced Practice Registered Nurse (APRN) to join
our direct care team on a full-time (30 hours/week) basis. We also
have a per diem opening as well. The APRN will focus on addressing
the complex medical and behavioral health needs of the uninsured,
underinsured, those experiencing homelessness and housing insecure
individuals by utilizing the nursing process to conduct
comprehensive person-centered assessments and identify co-occurring
medical, mental health, and substance use
disorders.Responsibilities - Provide patient-centered care that is
sensitive and responsive to the demographics, cultural background,
and specific needs of the population served. - Conduct thorough
physical examinations for patients across the lifespan. - Collect
and document comprehensive health and psychosocial histories,
including review of systems, in accordance with electronic medical
record (EMR) standards. - Order and interpret appropriate
diagnostic tests to assess and monitor health status. - Analyze and
integrate findings from health histories, reviews of systems,
physical exams, and diagnostic results. Share test outcomes with
patients during care visits. - Identify medical and behavioral
health concerns as well as educational needs of the patient. -
Develop individualized care plans that include health education,
promotion strategies, and disease management. - Implement and
monitor treatment plans for acute and chronic health conditions in
collaboration with the care team. - Initiate emergency
interventions in critical or life-threatening situations and
consult or refer to physicians or emergency services as appropriate
to stabilize patients. - Maintain strict confidentiality regarding
patient information, staff, legal matters, and organizational
operations. - Accurately code and document all patient encounters
in the EMR and ensure timely submission of billing information in
accordance with billing guidelines. - Maintain open and effective
communication with other healthcare professionals to ensure
coordinated, holistic patient care-both during clinical sessions
and formal care planning meetings.Requirements - Master's Degree or
Doctorate Degree in Nursing from an accredited program, required. -
Licensed as an Advanced Practice Registered Nurse in the state of
New Hampshire. - Minimum 2 years' experience as a mid-level
provider. - Prior experience working in community health,
behavioral health, or substance use treatment settings and
experience working with underserved populations, including
individuals experiencing homelessness or housing instability,
preferred - Basic Cardiac Life Support (BCLS) certification,
required. Advanced Cardiac Life Support certification, preferred. -
Valid Driver's License or the ability to transport self to and from
