We are looking for an RN to help coordinate a smooth transition from our hospitals to our skilled nursing facilities.Position SummaryWorking in the Acute Care hospitals, the SNF Admission RN has the responsibility and accountability to collaborate with Central Intake (CI), and SPHP Care Coordination staff to ensure a smooth transition of patients from acute care to the SPHP skilled nursing facilities. The position will work with CI and Care Coordination to ensure that patients admitted to SPHP skilled nursing facilities are clinically appropriate and that timely, complete and useable information follows the patient from acute to post-acute care.Schedule: 8-4p M-FLocation: St. Peters Hospital - Albany New YorkEducation RequirementsGraduate of a Registered nursing schoolCurrent New York State registration as a Registered Professional NurseExperience Requirements1-year experience in sub-acute or long-term care preferredWorking knowledge of Point Click Care (PCC) and EPIC EHRsStrong clinical assessment and care planning skillsComprehensive understanding of the SNF admission process and associated regulationsDemonstrated interpersonal skills to effectively communicateHigh level prioritization skills.Ability to work in an autonomous organized, creative and collaborative mannerKnowledge of interdisciplinary team approach to care management and nursing home regulations preferredPay range: $33.10-45.75/hourPay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
