Care in Bennington is looking for full-time MD Practitioner with
possibility for part ownership in the future. Full time 40 hrs. M-F
34 hrs. scheduled patient care time, 6 hrs. admin/nursing home Call
1 day out of 6, as well as 1 weekend out of 6 CME- $1500 allowance,
as well as 5 days paid CME time CTO 4 Wks. No hospital duties, or
on call Partnership (Owner) Opportunity/Consideration after 2
years. PCHP currently has 24 Partner MD Owners. Medical
specialties: Primary Care Family Practice Responsibilities: Provide
comprehensive medical care to patients of all ages, including
preventive care and treatment of acute and chronic illnesses.
Utilize EHR systems for accurate patient documentation and
management. Collaborate with specialists in areas such as
pediatrics, psychiatry, and endocrinology to ensure holistic
patient care. Perform and interpret medical imaging as needed and
engage in perioperative care when applicable. Stay updated on
advancements in family medicine and participate in continuous
professional development. Mt. Anthony Primary Care is an active
participant, along with most of the primary care community in
Bennington, with the Vermont Blueprint for Health , an initiative
of the Vermont Department of Health & the Office of Vermont Health
Access to promote practice redesign to more efficiently manage
chronic diseases and achieve better outcomes. Since 2009, we’ve
worked together in a state-wide collaborative to develop our
practices into Patient-Centered Medical Homes (PCMH). This is a
model of primary care initially adopted by the American Academy of
Pediatrics about 20 years ago, that has also been adopted by the
American Academy of Family Physicians, other primary care groups,
and on a national level through the National Committee for Quality
Assurance (NCQA). We are proud to say that since we first applied
for recognition in 2008-2009, we have been recognized by NCQA as a
Level 3 PCMH, which is the highest level, and have continued to
achieve recognition with each yearly renewal. In addition to
collaborating with other local, regional and state Blueprint
practices, we have also participated in collaboratives for
office-based opiate treatment (OBOT), i.e., buprenorphine,
sponsored by the addiction medicine department at
Dartmouth-Hitchcock Medical Center and involving practices from all
around the state. We use state of the art monitoring equipment for
our patients, such as, Holter Monitors, Computerized Blood Glucose
monitoring, Continuous Glucose monitoring, and Ankle-Brachial Index
measuring. Since Vermont developed the Blueprint and started
collaborating, Vermont has rapidly risen to one of the Healthiest
States in the Nation. Since 2003 Vermont has been among the
healthiest states in terms of clinical care, healthy behaviors and
health outcomes. Nestled in the Green Mountains of Southern
