Job Description We are currently offering a $4,500 Sign-On Bonus
for this position. /n Sigma Health Rehab is actively seeking a
Full-time Physical Therapist (PT) to work in a Skilled Nursing

Program. /n Key Responsibilities Include: /n /n - /n Evaluate
residents’ conditions & develop appropriate physical rehabilitation
plans /n /n - /n Relieve resident’s pain using exercises, hands-on
therapy & stretching /n /n - /n Help residents through their tasks
and monitor their performance /n /n - /n Supervise the activities
of physical therapy assistants /n /n - /n Maintain resident
confidence & protects facility by keeping information confidential
/n /n - /n Complies with federal, state, and local legal and
professional requirements /n /n - /n Maintains a cooperative
relationship among healthcare team members & contributes to team
effort /n /n /n PT Requirements Include: /n /n - /n Graduate of an
accredited Physical Therapy Program /n /n - /n Current New York
State PT License /n /n - /n Experience in a skilled nursing or
geriatric care setting is preferred, but new graduates with
relevant fieldwork experience are encouraged to apply /n /n /n ·
Current evidence-based practices consistent with the standards and
recommendations of the American Physical Therapy Association /n
