Community-Based Recovery Facilitator -Medication Management Specialist

Job Description:

Actively work with persons in the community to promote independence, skill building, and attainment of personal goals during their journey to recovery with hope and optimism.. Participate in an integrated team support model. .

Essential Job Functions

  • Keep daily record of time spent with Persons through daily documentation of contact and provide weekly updated schedules to the Office Manager
  • Work with Person to incorporate Treatment Plan goals in all contacts
  • Attend scheduled meetings with Persons, advocate and assist in developing and implementation of Person's treatment goals.
  • Participate in all relevant staff meetings, recovery team meetings, scheduled supervisions, trainings, and interagency meetings
  • Report any incidents, accidents, injuries or medication errors to your supervisor according to agency policy


  • Coordinate with doctors regarding any and all medication changes and/or concerns.
  • Provide Medication Administration to Persons per MAP protocol. This may include medication administration to cover staff emergencies and coverage needs.
  • Audit medications as assigned.
  • i.e. supported decision making.

  • Complete all required medication forms and assist the person at medical and psychiatric appointments,
  • Coordinate with medical providers to ensure accurate medication orders, signatures, clarification, reasons for medications, etc.
  • Responsible for ordering medications for persons served.
  • Review packaging plans and dispense medications according to each plan.
  • Coordination with outreach workers regarding medical and psychiatric appointments, ensuring med sheets are completed prior to the appointment.
  • Communicate events and potential risk to the site supervisor, clinical lead LPHA and/or assigned designee.
  • Complete and/or participate in the development of all required documentation (including medication assessments)
  • Communicate a sense of hopefulness and respect for Persons rights
  • Follow all agency and program policy and procedures
  • Complete all required trainings within four months of hire or program transfer (MAP, CPR, First Aid, Human Rights, Trauma Informed Care, Housing First, Mutuality, Orientation, Motivational Interviewing)
  • Represent the agency in a professional manner in all community contacts
  • Demonstrate a commitment to the Agency's mission and community mental health principles
  • Work cooperatively with staff
  • Other duties as assigned by supervisor


  • GED/H.S Diploma, Associates or bachelors degree in related field preferred, or 3-5 years of human service experience and two years of MAP experience.
  • Proficient computer skills and knowledge of Microsoft office, including excel
  • Knowledge of recovery model oriented curriculums {DBT, WRAP) desired
  • Excellent oral & presentation skills
  • Knowledge of community resources
  • Ability to conduct yourself professionally and manage confidential information

Other Requirements:

  • Ability to pass a CORI (Criminal Background check) and OMV (Department of Motor Vehicles check)
  • Able to use personal vehicle for business use
  • Must pass MAP training, CPR, and First Aid
