General Description:

The BHCP ACCS Liaison will be responsible for coordinating care for clients who are dually enrolled in the Behavioral Health Community Partner (BHCP) and ACCS (ACCS) Programs by understanding and communicating client needs across programs, based on staff and client contact as well as documentation in the BHCP and ACCS systems. The BHCP ACCS Liaison  will be responsible for coordinating: referrals from ACCS to BHCP; the assessment and care plan development process across programs; DMH-related input; Transitions of Care (TOC); medication reconciliations; and any other information or tasks that enhance coordination of service delivery across the BHCP and ACCS programs for dually enrolled Brien Center clients.

Essential Job Functions

  • Identify and track coordinated service delivery efforts for all clients who are dually enrolled in ACCS and BHCP
  • Identify and support referrals from ACCS to BHCP
  • Track, monitor and promote completion of BHCP engagement in the program for all referrals from ACCS
  • Collaborate with the BHCP Engagement Specialist in arranging and executing on collaborative efforts to enroll the client in BHCP with help from ACCS
  • Promote collaboration and coordination of service delivery for clients who are dually enrolled in the ACCS and BHCP programs among Brien Center staff
  • Pre-populate data on the Consent, Participation, Assessment and Care Plan forms using all available ACCS data in eHana 
  • Identify and transfer relevant information from ACCS and BHCP medical records, ensuring consistency of documentation regarding client needs, goals, care plans and coordination of services across programs in ACCS and BHCP eHana systems
  • Track assessment and care plan completion and coordinate efforts to decrease days to completion  
  • Identify and share relevant client information between BHCP and ACCS staff regarding engagement activities, assessments, Care Plan development, TOCs and medication reconciliations
  • Participate in warm handoffs to the care team once a client has completed an assessment for continued care coordination between BHCP and ACCS
  • Participate in quality assurance monitoring for metrics and processes related to coordination of service delivery across the ACCS and BHCP  
  • Contribute to the development and implementation of administrative processes to facilitate efficient and effective workflow(s) across the ACCS and BHCP teams
  • In collaboration with BH CP interdisciplinary team, identify cultural and linguistic needs and preferences and support the ACCS and BHCP teams in their efforts to deliver person-centered care based on client needs and preferences
  • Schedule, attend and manage follow-up tasks, as appropriate, for joint client planning collaboration meetings between ACCS and BHCP staff
  • Ensure notifications to ACCS and BHCP staff for dually enrolled clients occurs (new activity and updates to existing notes)
  • Track TOCs and Med Recs for dually enrolled clients; notify ACCS and BH CP staff and arrange joint planning and participation in arranging for client needs
  • Attend monthly DMH meetings; take notes and track and ensure completion of follow-up items for all dually enrolled clients whose cases are discussed including referrals and for existing clients
  • Obtain and evaluate data including, but not limited to: Total ACCS enrollment; BHCP enrollment; Total dual enrollment; Status of all referrals from ACCS

Other Responsibilities

  • Support and maintain the principles and policies of the agency and BCHP and ACCS Programs
  • Attend meetings as scheduled or requested
  • Flexibly respond to program scheduling needs
  • Promote collaboration, whenever possible, to support service delivery for clients enrolled in ACCS and BHCP
  • Communicate a sense of hopefulness and respect for enrollee rights and for colleagues
  • Represent the agency in a professional manner in all community contacts
  • Demonstrate a commitment to the agencys mission and values.
  • Drivers license checks (RMV).
  • Criminal Offender Record Information Check (CORI).
  • Attend and complete trainings as required.

Qualifications

  • Bachelors Degree required
  • Experience in community-based behavioral health support programs preferred
  • Experience with coordination and promoting collaboration across programs or entities
  • Understanding of person-centered care coordination and care management for individuals with behavioral health needs

  • Excellent ability to perform or manage communications, documentation, time management and organizational needs
